Saw Palmetto at Twenty Milligrams: What the Research Measured, and at What Dose
Saw palmetto is the first line on the VapoFil panel, at 20 mg a serving. It is also one of the most studied herbal extracts sold in the United States, which makes it an unusually good test case for reading a label: the research is large, carefully designed and easy to compare. This post goes through what that research measured, at what dose, and what it can and cannot tell a buyer of a 20 mg line.
- The saw palmetto research is mostly about urinary symptoms in men over 45 with an enlarged prostate, not about libido.
- The trials treat 320 mg a day as the standard dose; the CAMUS trial went up to 960 mg a day and still matched placebo.
- The 2023 Cochrane review of 27 studies found little to no difference from placebo in urinary symptoms.
- On sexual function, the evidence says saw palmetto does no measurable harm. It does not show a boost.
- VapoFil’s line is 20 mg, one-sixteenth of the standard trial dose.
What saw palmetto is, and why men take it
Saw palmetto, Serenoa repens, is a low fan palm that grows across the south-eastern United States. Its dark berries are dried and extracted, usually with alcohol or another solvent, into the oily extract that goes into capsules. The berry extract is what the research used, and the berry extract is what the panel prints.
Men take it mainly for one reason: urinary symptoms as the prostate enlarges with age, the getting-up-at-night and weak-stream problems doctors group as lower urinary tract symptoms. Because the prostate and sexual health sit close together in how men think about their bodies, saw palmetto has travelled into many men’s formulas sold for vitality and libido.

What the big trials measured
The best single trial is CAMUS, run at 11 North American sites and published in JAMA. The CAMUS dose-escalation trial randomised 369 men aged 45 and over with moderate urinary symptoms to saw palmetto extract or an identical placebo. The dose started at the standard 320 mg a day, doubled to 640 mg at 24 weeks and rose to 960 mg at 48 weeks, and the trial ran for 72 weeks in all.
Symptom scores fell in both groups: by 2.20 points on saw palmetto and by 2.99 points on placebo. Saw palmetto was no more effective than placebo on the main measure or on any secondary one, and the secondary measures included sexual function. Tripling the dose did not change the answer.
The broadest summary is the 2023 Cochrane review, which pooled 27 studies and 4,656 men, most of them over 50 with moderate symptoms. Its conclusion, rated high certainty, is that saw palmetto on its own makes little to no difference to urinary symptoms or quality of life compared with placebo, in the short term and over 12 to 17 months.
| Study | Who took part | Daily dose | What it found |
|---|---|---|---|
| CAMUS, 2011 | 369 men aged 45+ with urinary symptoms | 320, then 640, then 960 mg over 72 weeks | No better than placebo, sexual-function measures included |
| CAMUS safety, 2013 | 357 men from the same trial | Up to 960 mg for 18 months | No evidence of toxicity at up to three times the usual dose |
| Cochrane review, 2023 | 27 studies, 4,656 men | Mostly standard extract doses | Little to no difference from placebo in urinary symptoms |
| Sexual-function meta-analysis, 2021 | 20 papers | Various | No significant difference from placebo or tamsulosin |
| VapoFil label | Adults 18 and over | 20 mg in two capsules | One of seven lines in a formula sold for libido and stamina |
Doses are of saw palmetto extract. The trials enrolled men with prostate symptoms; none tested saw palmetto as a libido supplement.
The sexual-function question
The sexual-function research on saw palmetto was mostly asked from the opposite direction. Prescription drugs for an enlarged prostate can have sexual side effects, and a belief has circulated among patients that saw palmetto does too. A 2021 meta-analysis of saw palmetto and male sexual function set out to test that worry. It gathered 20 papers, eight comparing saw palmetto with placebo and seven with the prescription drug tamsulosin, and found no significant difference in sexual function in either comparison.
Read carefully, that is reassuring and modest at once. It says saw palmetto is unlikely to harm sexual function. It does not show that saw palmetto improves it, and CAMUS, which measured sexual function directly at up to 960 mg a day, found no difference from placebo either way.
The research answers whether saw palmetto hurts sexual function. It mostly does not ask whether it helps.Govapofill Editorial Desk

Twenty milligrams against three hundred and twenty
The arithmetic is short. The standard trial dose is 320 mg a day; the VapoFil panel prints 20 mg a serving. That is one-sixteenth of the standard dose and one forty-eighth of the top CAMUS dose. Reaching 320 mg from this bottle would take 32 capsules a day, and the label is clear that no one should exceed its two.
So the honest way to read the line is as a small traditional ingredient in a seven-part formula, not as the reason to buy it. The ingredients page sets every panel amount beside the dose its research used, and the case for VapoFil rests on the formula as a whole and on how buyers find it, which this desk reports on the reviews page.
A small dose has one plain advantage: a small exposure. That matters for the safety section below.
Saw palmetto in combination formulas
VapoFil is a combination product, so there is a fair follow-up question: does saw palmetto do better alongside other plant extracts? The Cochrane review asked exactly that. Alongside its 19 single-ingredient trials it pooled 8 trials of saw palmetto combined with other plant extracts against placebo.
The answer was less certain rather than more encouraging. Combinations that include saw palmetto may make little to no difference to urinary symptoms over 12 to 24 weeks, the reviewers rated that evidence low certainty, and they described more uncertainty about combinations than about saw palmetto alone. None of the combinations studied was this formula, and all of them were tested for the prostate rather than for libido.
So the combination question stays open, and it stays open for VapoFil as for most multi-ingredient products, because the trials were run on ingredients and on other formulas rather than on this one. That is why this desk reports what buyers say at four to eight weeks, and why it prints every amount so the formula can be read line by line.
Safety, interactions and the PSA test
Saw palmetto has a good safety record at far higher doses than this. NCCIH’s saw palmetto fact sheet describes it as well tolerated, used safely in studies for up to three years, with side effects that are mild and infrequent: digestive symptoms, dizziness and headache. The CAMUS safety report followed 357 men for 18 months on up to 960 mg a day and found no difference from placebo in adverse events, vital signs or laboratory tests.
Two practical points remain. Saw palmetto appears in the interaction literature for blood thinners, so anyone taking one should ask the prescriber before starting any product that contains it. And it is one of the supplements examined in a review of dietary factors and supplements that influence PSA results, so tell whoever orders a PSA blood test that you take it.

What this means for a VapoFil buyer
- Saw palmetto is one of seven ingredients in VapoFil, not its headline. Judge the formula as a whole.
- If you already take a saw palmetto product for urinary symptoms, the 20 mg here adds little, but count it in your total.
- If you take a blood thinner, ask the prescriber before starting.
- If you have urinary symptoms, get them checked. An enlarged prostate is common, and other causes are worth ruling out.
- Tell whoever orders a PSA test that you take it.
The side effects page covers the other six ingredients the same way, and the Supplement Facts page transcribes the panel word for word.
VapoFil is a dietary supplement, not a treatment for erectile dysfunction or any other condition. A lasting change in erections or desire can be an early sign of something that needs a check-up: an umbrella review of the research found erectile dysfunction consistently linked with later heart disease, and MedlinePlus lists the other causes worth ruling out, medicines among them. That is a reason to see a clinician first, not a reason to start a capsule.
